    All payroll issues that have not been disputed in Flaik must be submitted via this form. Emails sent to individual staff members will not be accepted.

    Please note: Pay issues not disputed in Flaik on day of pay may take up to 28 days to be resolved.

    You Must Submit One Form For Each Day Or Missing Paid Activity (Lesson, Support, Lineup, Training Etc.)

    This section is ONLY for pay rate or payroll classification issues.

    🚫 DO NOT select a rate-related option if you are missing pay entirely.🚫 

    If you are missing pay, use Missing Pay and enter each missing time block in the form.

    Issue With Private Request Rate
    Select this if you taught a Private Request lesson and were not paid the correct Private Request rate.

    Examples:

    • You taught a Private Request but were paid at a regular private or teach rate
      The Private Request premium was missing or incorrect

    Issue With Regular Teach Rate
    Select this if you were paid incorrectly for a regular (non-request) lesson, such as an adult group, kids group, or standard private lesson.

    This includes situations where:

    • You received a raise (such as PSIA/AASI certification) and do not see it reflected in Flaik or Paycom
    • Your base teach rate is incorrect for lessons worked
    • Do not use this for Private Request or Training pay issues.

    Issue With Training Rate
    Select this only if you attended a training, clinic, orientation, or required meeting and the training rate itself was incorrect.

    Examples:

    • You were paid for the training, but at the wrong rate
    • A training differential or approved rate was not applied
    • Do not use this if you are missing hours — missing time blocks must be entered directly in the form.

    Other Rate Issue
    Select this if your pay rate issue does not fit any of the categories above, but is still related to being paid at an incorrect rate.

    Missing Pay
    Select this if you worked scheduled time and received no pay at all for that work. Submit One Form For Each Day

    📌 The form will allow you to enter each missing time block individually, so please list every block of time "Teaching Task, Support, Training" that is missing on each partcular day.

    Paid At Incorrect Rate (i.e. Teach Instead of Private Request)
    Select this if you were paid, but the work was classified incorrectly.

    Examples:

    • Paid Teach instead of Private Request
    • Paid Training instead of Teach

    Missing or Destroyed Check
    Select this if a paper paycheck was:

    • Lost
    • Never received
    • Destroyed or damaged
    • This option is not for direct deposit issues.
