Conduct Statement:
As a parent/guardian, I understand that I, my family, and friends are expected to conduct ourselves in a civil manner at all SLB events and functions. This includes refraining from Obscene language, Yelling at referees, coaching staff, or other guests.
Failure to comply may result in removal from the event and/or possible removal from the SLB organization upon Board approval.
Fundraisers:
I understand that each player is required to participate in one mandatory fundraiser. Failure to fulfill the entire amount may result in my child not receiving a trophy, finishing the season, participating in playoffs, All-Stars, or end-of-season festivities.
Volunteer Requirements:
If I am not a certified coach or team parent, I must provide 2 volunteer hours of concessions, setup, or cleanup. Alternatively, SLB accepts a $50 alternative fee per hour. Failure to participate may result in my child not being allowed to finish the season.
Photos:
I grant the Somerset Lil' Bulldogs permission to take photos of my child for website and event purposes.
Refund Policy:
NO REFUNDS will be given. Full payment must be made at the time of registration.
I, the undersigned parent or legal guardian, have read and agree to the above conditions. By signing below, I acknowledge that I and the above-named player have given up substantial rights, granting permission for participation in the Somerset Lil' Bulldogs Football programs.
I understand that all coaches and team parents are volunteers. I agree to follow and abide by all rules and regulations provided to me.
I sign this form for myself and on behalf of the player, agreeing to these terms freely, voluntarily, and without inducement.
