Thank you for your interest in nominating someone for the FPM Volunteer Award. Please read the notes below before completing this form.
Eligibility
Nominations are accepted from current FPM members, FPM staff and lay members serving as volunteers (e.g. as a lay member of a committee). All nominations must be supported by a seconder.
How to nominate
Anyone who has done voluntary, unpaid, work for FPM can be nominated for this award. To nominate, you need to submit a supporting statement of between 300 and 1,000 words in length, detailing the nominee’s contributions to the Faculty and its mission.
Important notice:
Please note that nominees will be notified of their nomination (but not given the name of the person who nominated them) and permission will be sought for their name will be published on our website and in our newsletter.
How are the winner's chosen?
The Fellowship and Awards Committee will review all nominations on a case-by-case basis. Only the information submitted by on this form will be considered and any conflicts of interest will be declared at the beginning of the review process.
The committee will judge each nomination on its individual merit, without reference to any specific criteria, with the final decision agreed by a vote. The nominated awardee will be presented to the Board for final approval.
What do the winner receives?
The winner will receive an award and will be invited to our Annual Awards Ceremony for formal commendation of their achievements amongst their peers.
FPM Annual Awards Ceremony
If your nominee is awarded the Volunteer Award, both you and the nominee will be invited to attend our Annual Awards Ceremony on Wednesday 2 July 2025 at the Merchant Taylors’ Hall, London.
As a proposer, you will be asked to deliver a short citation for them. If you are not able to attend the ceremony, the person supporting this nomination may present the citation on your behalf.
