The information contained in this application is used only for the purpose of placing a puppy and will not be used elsewhere. A $1000 deposit to hold your puppy will be collected after your application is approved, and will be deducted off the total price of the puppy.
•All applicants must be 18 years or older

Are you looking for a pet or to add to an established breeding program? Please note, pet means the puppy is never allowed to breed and we do withhold AKC paperwork until proof of spay/neuter. Breeding rights is on a case by case basis.
Have you done your research on your preferred breed? What do you know about the breeds needs and health?
What socialization and training do you plan to implement?
Do you have a veterinarian you plan to use?
How often do you/will you take your dogs to the vet?
Do you have a fenced yard?
For the safety of your puppy, when not in an enclosed fence area, please keep your puppy on a leash at all times.
Will the puppy live indoors?

How many hours in a day would the puppy be left alone?
On average a puppy shouldn’t be alone more than 5 to 6 hrs. a day
Do you understand the time and energy it takes to care for a puppy:
Will you be attending obedience classes with your puppy?
Will the puppy be picked up?
Would you like to have the puppy delivered? This is an extra fee.
Puppies can be shipped anywhere in Canada and the USA.
Please bring a crate with you if you are picking up your puppy.
Do you agree to contact OMK in the event you cannot keep your puppy?
Do you agree to sign OMK health guarantee? This is required at pick up or before the puppy is transported. No puppy will be released without a signature from the buyer.
Please indicate anything else that may help us with selecting the right puppy for you.
After 10 weeks of age, unless discussed and approved prior, there will be a boarding fee of $15 a day up to 30 days. Board must be paid weekly. If not picked up by 16 weeks all funds are null and void and OMK can resell the puppy. Do you understand and agree?
