Reborn Ranch Camp Application

Parents, please take a moment to describe your camper's strengths as well as areas where they could grow. Use the space below to share as much detail as you can. At camp, we will focus daily on character development. Some examples of the traits we will work on include perseverance, learning and understanding, diligence, obedience, brotherly kindness, wisdom, helpfulness, thankfulness, and self-control.

Please attach a recent photo when submitting the application. Providing a photo of the camper helps our staff recognize the camper and warmly welcome them to camp. We will also be praying for God to work in their lives and to give them direction for their life through His Word.

By signing below, I agree to follow all camp rules and guidelines set forth by Reborn Ranch. I understand that these rules are in place to ensure the safety, well-being, and positive experience of all campers. I acknowledge that failure to follow these rules may result in dismissal from the camp program.

By signing below, I agree that my child will adhere to all camp rules and guidelines set forth by Reborn Ranch. I understand that these rules are designed to ensure the safety, well-being, and positive experience of all campers. I acknowledge that failure to follow these rules may result in appropriate actions to maintain the safety, well-being, and positive experience of all participants, including dismissal from the camp program if necessary. I further understand that, in the event of dismissal, I as the parent or guardian will be responsible for arranging and covering the cost of transportation for my child.

By signing below, I affirm my understanding of and agreement to these terms and conditions. As the parent or legal guardian, I agree to the following: I release, indemnify, and hold harmless Reborn Ranch, Inc., its board members, staff, volunteers, agents, and affiliates from any and all claims, actions, liabilities, and damages arising from or related to my child’s participation in camp activities, including but not limited to horseback riding, swimming, life skills training, community service projects, and other outdoor or off-site activities. This release applies regardless of whether such claims arise from negligence or other causes. Additionally, I agree that if my child causes intentional or negligent damage to property, equipment, or facilities at Reborn Ranch or during any off-site activities, I, as the parent or guardian, will be held financially responsible for such damages. I confirm that all the information provided in this application is accurate to the best of my knowledge. I certify that my child is in good physical and mental health and is capable of safely participating in all camp activities. I will communicate any health concerns or limitations in writing and include the information with the camp application. I consent to my child’s participation in activities involving animals and equipment, understanding the inherent risks, including but not limited to falls, animal unpredictability, and equipment-related accidents. I acknowledge that safety instructions will be provided, and I will discuss the importance of adhering to them with my child. I authorize Reborn Ranch staff to make decisions in the best interest of my child’s safety, including emergency medical treatment, transportation to a medical facility, and participation in off-site activities under supervision. I understand that life skills training and community service projects are integral parts of the camp program and may involve leaving the camp facility. I consent to my child’s supervised transportation for these purposes. I acknowledge that Reborn Ranch integrates Christian principles and Biblical teachings throughout the camp program and give my consent for my child’s participation in these faith-based activities. I understand that photographs and videos of my child may be taken during camp activities for use in promotional materials, including brochures, online platforms, and other literature. If I do not wish for my child to be included, I will provide a written opt-out statement with this application.
