Aftercare is also provided on artists website (www.redeemedinkandco.com)I understand results vary from person to person.I understand one or multiple touch ups may be needed after healing. First touch up is complimentary and any additional touch ups needed are at an additional cost.I am NOT pregnant or breastfeeding at time of scheduled appointment.I certify that I am 18 years of age or older.I agree that the design is drawn to my satisfaction and if uncomfortable with design, artist or establishment that I'm encouraged to have it redesigned or decline tattoo.I confirm that the information I provided in this document is accurate and true.I hereby release the artist at Lisa Marie Oliver LLC, the establishment DBA Redeemed Ink & Co and the owner of all legal responsibility of the tattoo and health of the client.
