Special License Plate Series Signature

Before the Department of Licensing (DOL) considers the Donate Life Washington plate series, we require 3,500 signatures in support of this project. By signing this form, you're indicating your interest in purchasing a Donate Life Washington license plate when it becomes available, although there is no obligation to purchase one. Proceeds from the special license plate sales will fund public education and outreach about organ donation in communities all across Washington.
