Special License Plate Series Signature

The Department of Licensing (DOL) will produce the Donate Life Washington plate series when 3,500 signatures (or committed plate purchases) is reached in support of this project. By signing this form, you're indicating interest in purchasing a Donate Life Washington license plate when it becomes available, although there is no obligation to purchase one. Proceeds from the special license plate sales will fund public education and outreach about organ donation in communities all across Washington.
