If you haven't spent time discerning who your Saint and Sponsor should be for Confirmation, please pause and take some time to do that. Here are the instructions.

Attention parents: This form should be completed by your child, since some of the questions ask about why they selected this saint and asked this person to be their sponsor for Confirmation- and we want to hear about that in their own words! Please feel free to assist them with the remainder of the form, as necessary.

You'll need the following information to complete this form:
- A parent email address
- Your selected Confirmation Saint
- Your selected Sponsor's name & email address
- The name and city of the parish where your sponsor they attends Mass
- Whether or not your Sponsor received any Sacraments at St. Patrick, Brighton

Please use this field to tell us which Sacraments they received at St. Patrick. If possible, include they year they received it:

We will need to collect a copy of your sponsor's baptismal record (certificate) by May 1, 2025.
- If they do not have a copy of their baptism certificate at home, they need to contact the parish where they were baptized to request a copy. A photo or scan of their baptism certificate may be submitted to us via this form.
- Alternatively, your sponsor can ask the parish of baptism to send the certificate directly to us here at St. Patrick: ATTN Marissa Dawson, 711 Rickett Rd, Brighton MI 48116.
We typically collect a copy of each Confirmation Sponsor's baptism record (certificate). However, since your sponsor received one or more Sacraments at our parish, we may have theirs on file, already.
- If your sponsor was baptized at St. Patrick, we are the official holder of their record and definitely have it on file. You will not need to turn anything in for them.
- If your sponsor received First Communion or Confirmation here at St. Patrick, we may have their baptism record on file from when they did their preparation for it. In this case, we will check our files based on the information you provided and let you know if you need to take any further action to provide us with their certificate.
