Agreement
I, the abovesigned, hereby grant permission to Kevin Polowy, Addison Henderson, AppleBox, and their affiliates, representatives, and assigns (collectively referred to as "Producer") to use my name, likeness, image, voice, and any statements made by me (collectively referred to as "Footage") in connection with the production and promotion of the docuseries titled "Just One Before I Die."

1. Grant of Rights
I hereby grant the Producer the right to record, edit, and distribute the Footage in any and all media now known or hereafter devised, worldwide, in perpetuity, for any purpose, including but not limited to promotional materials, social media, broadcast, and online streaming.

2. No Compensation
I acknowledge that I will not receive any compensation, now or in the future, for the use of my Footage as described above.

3. Release of Liability
I release and discharge the Producer from any claims, demands, or liabilities arising out of or in connection with the use of the Footage, including but not limited to any claims for invasion of privacy, defamation, or copyright infringement.

4. Governing Law
This agreement shall be governed by and construed in accordance with the laws of New Zealand and the laws of the United States of America.

5. Understanding of Terms
I confirm that I have read this Footage Release Form and fully understand its contents. I am of legal age and have the right to enter into this agreement.

6. Signatures
By signing above I agree to the terms outlined in this Footage Release Agreement.
