ENROLMENT CONDITIONS AND AGREEMENT

1. 10 Class Pass is valid for 6 months from date of purchase. Class pass is valid for Adult Ballet (Thursdays) & Adult Jazz (Mondays) - Both classes are 1 hour per class. Both classes are at The Academy's Sturt Campus (Level 1, 1 Sturt St, Ballarat Central (next to Pancake Kitchen) and operate during School Terms.

2. Punctuality and respect for others is expected and members are enrolled/registered to enjoy the training/environment at The Academy.

3. The Academy of Classical Ballet takes no responsibility for loss or damage to personal property.

4. In case of any emergency, if emergency contact cannot be contacted, The Academy will act accordingly with any costs incurred bourn by the registered student/dancer.

5. Student/Dancer may be photographed/videod and such media used as required by The Academy of Classical Ballet.

6. Student/Dancer hereby accept all Rules, Regulations, Policies and Requirements set by The Academy (all available on our website).

7. Student/Dancer hereby agrees that The Academy of Classical Ballet is NOT responsible for any student/dancer at any time other than when they are in an enrolled class.

8. All details provided on this form will be used for The Academy Database and/or database of our partners or sponsors.
