COVID-19: I (Parent or Guardian) understand that Family Karate has implemented steps and procedures to reduce the risk and spread of COVID-19 in accordance with state and federal guidelines. However, due to the nature of the novel coronavirus, Family Karate cannot guarantee any visitor, guest, student or camper entering inside or outside of our facility will not be potentially at risk. By entering our facility, you hereby assume full responsibility and waive all claims against Kim Studio of Karate at Lebanon, Inc. D.B.A. Family Karate, its owners, staff, instructors, partners or otherwise, for any and all injuries, claims, medical expenses or damages.

By signing below I acknowledge that I have read the foregoing liability release waiver and understand its contencts ; that I am at least eighteen years old and fully competent to give my consent. ; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed: that I give voluntary consent in signing this liability release waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
