  • Scholarship Agreement
    {ScholarshipSelection}

    I, {studentName}, as the recipient of the Garber Buick $2,000 Drive Your Community Scholarship, agree to the following:

    1)    This scholarship is available to me only if I graduate from a high school in the Great Lakes Bay regional area (Saginaw, Bay, or Midland counties).

    2)    I will enroll in a university, college, vocational school, or a tuition-based program to pursue a degree or certification, within two years following my high school graduation.

    3)    I will complete and submit the Saginaw Community Foundation (SCF) Garber Buick $2,000 Drive your Community Scholarship Information Form to request the scholarship funds.

    4)    I will provide the SCF with a copy of my enrollment receipt for the school or program I enroll in.

    5)    I will immediately inform the SCF of any change in my address or telephone number between now and when I graduate from high school or receive my GED.

    6)    I will furnish the SCF with additional information upon request.

    7)    I give permission for any college, school, or tuition-based program to release to the SCF any information necessary to process my scholarship.

    I understand failure to abide by these conditions shall constitute sufficient reason for the termination of the scholarship.

    I, {studentName}, as recipient of the {ScholarshipSelection}, agree to the following:

    1)     I will maintain the enrollment status, major/program of study (if applicable), and GPA requirements as set by the scholarship qualifications listed in the SCF scholarship directory. 

    2)     I will attend the institution type (college/university/CTE) as specified by the scholarship qualifications listed in the SCF scholarship directory. This includes institution location requirements, as defined by the scholarship qualifications in the SCF scholarship directory.

    3)     I will send a copy of my receipt of enrollment and/or financial and school information form to the Saginaw Community Foundation to prove my registration at {CollegeName}. 
     
    4)     I will send a copy of my transcript to the Saginaw Community Foundation at the end of each semester as a record of my continued enrollment.
     
    5)     I will maintain the integrity of this scholarship by maintaining at least a minimum grade point average required by my school to avoid being placed on scholastic probation, and by upholding the good moral character that originally qualified me for this scholarship.
     
    6)     I will immediately inform the Saginaw Community Foundation of any change in my status as a student, in my academic major, or in my address or telephone number.  If I interrupt my college education for longer than the normal summer vacation period, I am aware that my scholarship may be terminated.
     
    7)     If I transfer to another college, I will provide the Saginaw Community Foundation with an official statement from that college indicating I have been granted admission.  I understand some scholarship grants are not transferable.
     
    8)     I will furnish the Saginaw Community Foundation with additional information upon request.
     
    9)     I give permission for any college or school to release to the Saginaw Community Foundation any information necessary to process my scholarship.
     
    10)     I hereby grant permission to the Saginaw Community Foundation and scholarship donors to use my name, photograph, and awards received, high school, institution, graduation year, or major/minor in publications and presentations.
