Kitsap Life Academy is committed to preparing children for life. We ask that there is an open dialogue between you and your child’s teacher to keep each other informed of significant happenings that may affect your child’s education and emotional well being, which are closely related. 

I have read all the terms and will uphold this agreement. 

Kitsap Life Academy is a supplemental education service. We are responsible for supporting academic learning and are providing an enrichement experience. Parents are responsible for their children’s education. The law and duties/responsibilities of a parent and a copy of the declaration of intent can be found on the Washington Homeschool Organization’s website or HSLD.org

I understand that I am responsible for my child’s education. That Kitsap Life Academy is not a school and is only enrichment to the education I provide. 

By signing below I agree to this. 

I hereby give my permission for my child to participate in the Kitsap Life Academy activities (Hereinafter referred to as “KLA”).

I hereby sign this agreement on behalf of my child.

I agree as a precondition to my child’s participation in all activities including, but not limited to, AM program, electives, animal interaction, outdoor educational activity, events, workshops and tours organized and/or conducted by the KLA staff. I agree to be strictly bound by the terms of this KLA Participant Waiver, Agreement & Release of Liability Form (hereinafter referred to collectively as “This Agreement”).

 I recognize the dangers of participating in farm activities (including, but not limited to walking paths, tours, interaction with animals and outdoor physical activities) including but not limited to, uneven ground, insect and animal bites/stings, and utilizing tools. I understand KLA is not responsible for any allergic reactions that may occur while on the property.
I also understand that outdoor activities may occur in the hot sun, in the cold, and in the rain. I agree to see that my child is appropriately attired for activities, and to provide insect repellent and sunscreen for my child to use. I will not expect the Kitsap Life Academy to provide these items. I give my permission for Kitsap Life Academy leaders to apply or assist with the application of the repellent and sunscreen I provide.
I acknowledge that interactions with animals involve inherent risks that may cause serious injury, illness and possibly death to participants.
In the event of illness, injury, and/or accident, I authorize the teacher or any Kitsap Life Academy employee to act on my behalf. They may approve any and all non-emergency treatment and are authorized to sign any and all medical release or required form(s) on my behalf. In the event of an emergency, I understand that I will be notified of the situation as soon as practicable. I agree to pay any necessary expenses not covered by the Kitsap Life Academy student accident policy incurred in the medical treatment of my child, including, but not limited to all transportation costs to and from a medical facility, and, if necessary, transportation to my home or medical facility of choice. 

I understand that the Kitsap Life Academy may, in its sole discretion, dismiss any participant for inappropriate, disrespectful, or dangerous behavior at any time. If my child breaks or damages any property as a result of their direct or indirect behavior, I hereby agree to pay for its repair or replacement.
In consideration of my application and permitting my child to participate in this activity, I hereby: WAIVE, RELEASE, AND DISCHARGE any and all claims, which I may have against KLA, and/or its Directors, staff, and/or farm host and land owners and release the KLA Directors, volunteers, staff, farm host and/or land owners from all liability for injury, death, property damage, property theft, or any other loss sustained by my child as a result of their participation in KLA, due to any cause whatsoever including, without limitation, negligence on the part of KLA or its staff members; INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the Kitsap Life Academy, its Board Members, officers, employees, volunteers, farm host, land owners or other entities or persons released from any and all liabilities or claims made as a result of participation in this activity. I further release KLA from any and all claims seeking to recover legal fees and/or costs of any type or nature.
I understand that the farm host and land owners assumes no liability for any accidents or injury during participation in Kitsap Life Academy.

 I understand that while participating in this activity, my child may be photographed. I agree to allow their photo, video, or film likeness to be used for any legitimate purpose by the Directors, Board and Staff. 

The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. Kitsap Life Academy, its directors, Board members, volunteers and all its employees, acting officially or otherwise are hereby released from any and all claims, demands, actions, or causes of action on account of any injury to my child that may occur. This release binds my heirs, executors, administrators, and/or assigns.
I understand, acknowledge and agree this release applies whether KLA is at fault or not and it releases not only the farm host and property owners but staff of KLA. I understand and acknowledge that KLA, in securing execution of this release and waiver, is relying upon my actions herewith as free and voluntary actions and my signature is not subject to any promise or inducement made to me by anyone.
I have read and understand this Agreement, I understand this document contains a promise not to sue Kitsap Life Academy, its Staff Members, Farm Hosts and Land/Property owners and release and indemnify the same for all claims.

 

Please note this agreement is considered valid from March 1, 2025 – July 31, 2026. 

Morning sessions will be broken up into groups by grade of child as of August just prior to the start of the year. However, the placement into groups is determined by the teachers. 

Tuition: The amount of tuition for a year is based on the amount we need to run the program and can be broken up into payments for the convenience of families. 

Sibling discount: 10% for each additional child

If a student is withdrawn following enrollment, the financial responsibility is as follows.

Time of Enrollment - July 31st: Amount Due is 25% of agreed upon tuition.

August 1st and on: Amount Due is 100% of agreed upon tuition. 

 

A copy of this agreement outlining your payment plan and ways to pay will be given to you after your child has been enrolled in KLA.
