Acknowledgment, Authorization, and Waiver
Waiver of Liability

I, [Participant's Full Name], hereby acknowledge and agree to the following terms and conditions in consideration for my participation in the Cape Youth Football Association Cheer Clinics:

1. Assumption of Risks: I understand that participation in cheer activities involves inherent risks, including but not limited to physical injury, illness, disability, or death. I voluntarily assume all such risks, known and unknown, arising out of or related to my participation in the cheer clinics.

2. Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Cape Youth Football Association, its officers, directors, employees, volunteers, agents, representatives, and affiliates (hereinafter referred to as "Released Parties") from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including but not limited to bodily injury, personal injury, or property damage, that I may sustain as a result of my participation in the cheer clinics, whether caused by the negligence of the Released Parties or otherwise.

3. Indemnification: I agree to indemnify and hold harmless the Released Parties from any and all liabilities, damages, costs, expenses, including reasonable attorney's fees, and claims arising out of or related to my participation in the cheer clinics.

4. Medical Treatment: In the event of any injury or illness that I may sustain during the cheer clinics, I authorize the Released Parties to obtain necessary medical treatment for me and agree to be financially responsible for any costs associated with such treatment.

5. Compliance with Rules and Instructions: I agree to comply with all rules, regulations, and instructions provided by the Cape Youth Football Association and its representatives during the cheer clinics.

6. Photographic Release: I grant permission to the Cape Youth Football Association to take photographs or videos of me during the cheer clinics and to use such materials for promotional or informational purposes without compensation to me.

7. Governing Law: This waiver of liability shall be governed by and construed in accordance with the laws of Florida, without regard to its conflicts of law principles.

8. Severability: If any provision of this waiver of liability is found to be invalid or unenforceable, such provision shall be severed from the remaining provisions, which shall remain in full force and effect.

I have read this waiver of liability in its entirety and fully understand its contents. I am aware that by signing this waiver, I am waiving certain legal rights, including the right to sue. I voluntarily agree to the terms and conditions stated herein.
