Consent and Acknowledgment to Bodywork

I have read and understood the below statement in regards to bodywork to be given to my horse(s) by Unbridled Hands Equine Massage, LLC. 

I undmerstand the this person is not a veternarian, that the bodywork given is not to be considered veterinary treatment and that comments, suggestions, or recommendations profferred in this course of this therapyare not to be construed asveterinary medical advice. Massage/bodywork therapy and any other holistic modalities are not a substitute for veterinary medical care, but rather they are a cooperative form of energy.

I wish to have massage/bodywork for my horse(s) and give my consent to and acknowledge by signing below

I certify that my horse(s) have received regular veterinary care needed.

I give my consent to have the massage/bodyworksessions recorded and have parts of the massage/bodywork used on social media platforms such as FaceBook, Instagram or for advertising purposes. 

In signing this document, I acknowledge that I have read and fully understand the previous statements and consent to Unbridled Hands Equine Massage Therapy, LLC to perform massage/bodywork on my horse(s). I understand that I am waving any and all claims that I may have against Unbridled Hands Equine Massage, LLC.
