FACILITY USE AGREEMENT

This agreement is made between the person hosting this event and Waldron Public Schools, concerning the use of a WPS facility.

The HOST agrees to comply with the following terms and conditions:

1. The HOST will pay the amount $50.00 as rent for the use of the facilities.

2. The HOST will also submit a damage/cleaning deposit in the amount of $50.00. This damage/cleaning deposit will be returned to the HOST if there are no damages to the facilities or the equipment and if facilities are clean. If there are damages caused to the facilites, the HOST will reimburse WPS for damges.

3. The HOST assumes all responsibility and liability for the requested use and agrees to defent, indeminify and hold WPS harmless from any and all loss, damage or injury to any person or property arising from use of the facilites.

4. The HOST agrees to clean facilities after use and remove all trash. If HOST fails to do so, HOST will reimburse WPS for teh cost of cleanup.

5. The HOST will be solely responsible to provide adequate surpervision of the activities conducted in WPS facilities.

6. The HOST will comply with all applicable state lawas and WPS policies.

7. Employees of WPS will not have to pay a rental fee if facilites are left clean, free of trash, and no damages have been made to facilities.
