Acknowledgment and Assumption of Risk. By signing below, I, the undersigned parent/guardian of the above-named participant, acknowledge and agree to the following:

Voluntary Participation: I understand that participation in the pickleball tournament is entirely voluntary and is not a required school activity.Risks Involved: I understand that this event involves physical activity, which may include risks such as, but not limited to, slips, falls, collisions, or other injuries.Assumption of Responsibility: I accept full responsibility for any risks, injuries, or damages that may result from my child’s participation in this event, whether caused by negligence or otherwise.
Release of Liability-In consideration of the acceptance of my child’s participation:

I hereby release, waive, discharge, and hold harmless Legacy Middle School, the Williamson County School District, Legacy Middle School PTO, event organizers, volunteers, staff, and administrators from any and all liability, claims, demands, actions, or causes of action related to injuries, damages, or losses arising from this event.
Parental Consent and Agreements

Medical Treatment Authorization: I consent to emergency medical treatment for my child if deemed necessary by the event staff or emergency personnel.Photo/Video Release: I grant permission for photographs or videos of my child during the event to be used for PTO or school promotional purposes.Behavior Expectations: I acknowledge that all participants are expected to adhere to Legacy Middle School rules, including good sportsmanship and respectful behavior. I understand that failure to comply may result in my child’s removal from the event.
Additional Information

Equipment Use: I understand that equipment will be provided by Legacy Middle School, but participants are allowed to bring their own equipment.Safety Measures: I acknowledge that adult supervision, administration, and a School Resource Officer will be present during the event.a question
