Vendors selling cosmetics must provide products that can be opened and tested. Note on the application ingredients used of BC origin. All ingredients must be listed on the product label. All products must be approved by Health Canada. Click here for the Canada Health guidelines. Click here for the form you need to submit to Health Canada. After submitting each of your products you will receive a Cosmetic Notification Number – it is those numbers you need to submit to the market. Here is more information about this process.

I certify that all products offered for sale and display will be made, baked, grown or raised by myself or under my direction/supervision within the defined "local" area of the Creston Valley Farmers' Market.

I have read the CVFM Rules of Operations and agree to comply with them (including any amendments made by the Market during the season provided notice of those changes is provided to me via contact information given). I also agree to follow all applicable requirements of the Interior Health Authority.

All information submitted on this form is to the best of my knowledge, true and accurate. The Market Manager reserves the right to remove from my stall any products not listed and pre-approved. If found to be otherwise by the Board of Directors, I recognize that disciplinary measures may be taken including expulsion from the market.
