    Welcome to the Vitals for Youth Ministry Online Assessment.

    Before you begin, we will ask for some basic information about you and your church. Questions about denominational affiliation and your church’s teaching and practice regarding the gifts of the Holy Spirit help us identify discipleship trends across different church traditions. These answers will not affect your assessment scores.

    We may report assessment findings in aggregate, but we will not publicly identify individual respondents or churches.
