I hereby give my approval for my child’s participation in any and all activities prepared by Beach Tennis Brisbane during the tournament. I assume all risks and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Beach Tennis Brisbane and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of travelling to, participating in, or returning from the event.

In case of injury to said child, I hereby waive all claims against Beach Tennis Brisbane, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including Beach Tennis.

In the event of illness or accident, I give my permission for emergency treatment by first aiders and by qualified medical personnel for my child, and I authorize the person in charge to take my child to a hospital or emergency facility. I give consent for the facility to secure any and all necessary emergency medical care for my child.
