PHOTO/FILM RELEASE:
By checking “yes” below, I hereby give Educational Resource Group LLC the absolute and irrevocable right and permission, with respect to video and photographs that its employees and volunteers may take of me during the program for which I am registering,
(a) to copyright the same in the name of Educational Resource Group LLC and/or Khyati Y. Joshi;
(b) to use, re-use, sublicense to other entities, publish and republish the same, in whole or in part, individually or in conunction with other photographs or images, in any medium, including but not limited to print, video, or the Internet, for all purposes including advertising, trade, or any commercial purpose throughout the world and in perpetuity.
I hereby release and discharge Educational Resource Group LLC, its officers, members, employees, agents, successors, and assigns, from any and all claims and demands arising out of or in connection with the use of such photographs, film, or tape, including, but not limited to, any claims for defamation or invasion of privacy.
I hereby consent to the release of said video, audio, film, photographs or any other medium for the above-stated purposes and in accordance with the terms stated above.
I am of legal age and have read the foregoing and full understood its contents.
