PHOTO/VIDEO RELEASE:
During the running of RMIB, staff members may take photographs and videos of various program activities and program participants for four purposes:

● Photos will be taken for future promotional use.
● Photos may be taken for training/educational purposes.
● Students and staff enjoy sharing pictures with each other in various formats including social media. First names are only used with no other personal information. This release provides consent for sharing.

Waiver of Liability and Hold Harmless Agreement

The Rocky Mountain Invitational for the Blind is an athletic competition for students ages 13-18 who are visually impaired, blind, or deaf held June 4-6, 2026.
1.     In consideration for receiving permission to participate in the Rocky Mountain Invitational for the Blind (hereinafter referred to as RMIB), I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT TO INDEMNIFY the agents, servants, volunteers, staff, or employees (hereinafter referred to as RELEASEES) from any and ALL liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child, or any of the property belonging to me or my child, or otherwise, while participating in the RMIB.

2.     I am fully aware of the potential risks and hazards connected with participating in or my child’s participation in RMIB, including but not limited to travel risks, interacting, riding, or being near horses, and/or medical or accident risks. I hereby elect to voluntarily participate or allow my child to participate in RMIB 2025 with my full knowledge that said activity may involve risk to me and my property or to my child and my child’s property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that my be sustained by me or my child, or any loss or damage of property owned by me or my child, as a result of being engaged in RMIB.

3.     I understand that I, as a Participant or Parent/Guardian of a Participant, should sustain or otherwise acquire and maintain an adequate insurance policy to cover any circumstance arising from my or my child’s participation in RMIB, or any activity associated with or facilitating that participation.

4.     It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind members of my family, spouse, my heirs, assigns and personal representatives, or those of my child and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above named RELEASEES in the case of any harm, injury, accident, or any damage to property. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Utah.

5.     IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least 18 years of age, and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same.

If Participant is under the age of 18, Parent/Guardian consents to the minor’s participation in the event and signs the agreement on their behalf.

Participation Fees

$35/athlete
The cost to your athlete is $35. Your athlete will be provided a t-shirt, dinner, and lunch. Accommodations and transportation for Utah athletes will be provided by the Utah Schools for the Blind. Outside of Utah, transportation and accommodations will need to be arranged by the parent/guardian or school district of the athlete.

Please make sure that your athlete has appropriate gear for the invitational, i.e. shoes, shorts, sunscreen, and they have been practicing for their individual events.

Parents can attend to support and watch their athlete, but all cost and transportation must be taken care of by yourselves. However, please understand that we are striving for your athlete’s independence, and act accordingly. For those in northern Utah, USDB will be providing transportation for the individual athletes only.
