    Below you will find 4 action items that need to be completed in order to be registered and ready for class! 

    • SERVICE AGREEMENT - signature required 
    • Services: SMS of Metro Detroit, LLC (the Trainer), shall provide dog training services to the client pursuant to the program and based on the terms set forth under the pricing sheet. All group training sessions are 4, 6 or 8 sessions, unless otherwise noted on the class schedule and client is responsible for attending each session. 

      Note: Due to AKC regulations, puppies attending 4 sessions are not eligible for AKC S.T.A.R award.

       
      General Health Policy: In the state of Michigan, a rabies vaccination is required on all dogs four months of age and older. Other vaccinations are given at the discretion of veterinarians. However, the following health protocols are considered to be a minimum when your puppy is coming into the facility for lessons or group class:

      For puppies, one set of the Parvo/Distemper combo shot after the puppy has attained 10 weeks of age (does not need to contain Leptospirosis)
      For puppies older than 16 weeks, a current Rabies vaccination, per state law unless veterinarian advised otherwise
      Flea/tick free

      Additional Family Members & Minor Children: Additional family members, including minor children, are welcome to attend puppy class provided they are not posing a distraction to other class attendees. Children may not run around the facility at any time. While minor children may work with the puppy, an adult is always expected to be actively supervising both the puppy and the child.

      Payment: The full training fee of client’s puppy class is due prior to provision of any instruction or a payment arrangement has been signed by the client. The full training fee is deemed earned upon delivery of the first group class. A charge of $50 shall be charged for any check returned unpaid by your financial institution for any reason.

      Cancellation/Refunds:  If client requests to cancel training more than 7 days in advance of the first day of class, a full refund will be granted less a 5% processing fee. For cancellations less than 7 days in advance, we will retain a $50 fee. Cancellations and refund requests after instruction has begun will be on a case by case basis and be less a 5% processing fee.

      Missed Classes & Class Forfeiture:  When booking your class, please let us know if you will need to miss a week of class. We will gladly extend out your session. However, we do not offer makeup classes, though we can handle extenuating circumstances on a case by case basis. We also offer a one on one consultation at the conclusion of class to go over any missed material and discuss a future training plan for your puppy.

      If you no call/no show two consecutive classes, we will assume you have dropped out of class and fill your space. Please keep this policy in mind when scheduling elective surgeries, such as spay/neuter, for your puppy.

      Indemnification / Liability: Client is responsible for the control, conduct and training of the puppy. Client is solely liable for any injury to persons, other puppies or damage to property caused by the puppy. For Puppy Classes, much of the training program may be conducted in groups exposing the puppy to other dogs. Client holds harmless and shall indemnify and defend trainer from any and all injuries or damage caused by the puppy. Client agrees that although trainer can provide instruction for the training of the puppy, the trainer does not maintain control of the puppy, and client is responsible for the successful training, control and conduct of the puppy during group training sessions, and at all other times. If the puppy is deemed aggressive by the trainer, and therefore unsuitable for puppy class, client shall be asked to leave, and other arrangements shall be made.

      Client agrees to release trainer from any and all manner of damages, claims, loss, liabilities, costs or expenses arising out of trainer’s services, except which may arise from the gross negligence or intentional and willful misconduct of the trainer. In addition, client will indemnify, defend and hold trainer and its employees harmless from and against any and all losses, liabilities, damages, fines, penalties and expenses (including attorney fees and veterinary costs and expenses) arising out of the performance of this agreement and otherwise related to any and all acts of behavior of client’s pet including but not limited to destruction of property, dog bites, injury and transmission of disease.

      Images: Trainer may photograph and/or video training sessions for future use in training, advertising or other purposes. Client consents to trainer’s use of images of client and the puppy in such manner.

      Results: All puppies have different temperaments and their owners vary in ability and willingness to follow the program; therefore, results will vary.

    • WELLNESS RECORD FOR MY PUPPY 
    • Please upload a copy of your puppy's wellness record from your veterinarian. While we don’t require specific vaccinations, we do need confirmation that your puppy is under the care of a licensed veterinarian before you can attend classes.

      📌Please note, we do not accept documentation from your breeder as proof of veterinary care.  Documentation must be from a licensed veterinarian.
