Parent Agreement and Release Forms
ATTENDEE AGREEMENT
In consideration for being accepted into the Dance Masters of America Student Honors Intensive Program the undersigned, jointly and severally, do hereby release Dance Masters of America, Inc., the Officers of the Organization; the Directors and Assistants of the SHIP Program; the faculty; The Ball State University, from any and all claims for damages, or for injuries which the Attendee may sustain while participating in any and all activities with the Dance Masters of America S.H.I.P. program.

*A DOCTORS NOTE stating the attendee is fit to actively participate in the program is REQUIRED at the mandatory check-in.

TRAVEL AUTHORIZATION

ONE of the following authorizations must be filled out.

Option #1 - Permission for child to travel alone, to and from the SHIP Program.

Option #2 - Permission for Dance Masters of America to release said child to a specific parent, guardian, or chaperone, at the end of the SHIP Program.
PARENTAL TRAVEL AUTHORIZATION

(Child Traveling Alone)

That I, Parent First Name Parent Last Name , am the parent and legal guardian of Student First Name Student Last Name, and do hereby give permission to travel without a chaperone or adult supervision to Ball State University to attend Dance Masters of America SHIP, arriving on July 19th, 2026, and to be released by Dance Masters of America on July 25th, 2026, without a chaperone or adult supervision to travel home from the SHIP Program.
PARENTAL TRAVEL AUTHORIZATION

(Child Traveling with a Parent or Chaperone)

I, Parent First Name Parent Last Name , the parent and legal guardian of Student First Name Student Last Name, do hereby acknowledge that said child shall be released to, First Name Last Name, at the end of the Dance Masters of America SHIP Program.

MEDICAL AUTHORIZATION

I, Parent First Name* Parent Last Name* , am the parent and legal guardian of Student First Name* Student Last Name*, and do hereby authorize Dance Masters of America, Inc., their agents, servants, and/or employees to obtain whatever medical and/or hospital care and treatment may be deemed necessary, within their sole discretion, while my child is attending the Dance Masters of America, Inc., SHIP, at Ball State University, from July 19th (arrival date) through July 25th, 2026 (departure date).

MEDICATION & PRESCRIPTION INFORMATION
List all illnesses and allergies of attendee: (If none, type "none")*
If YES, please explain illness:
Please list all the attendee’s prescription medications, non-prescription medication, over the counter medications, patches, pain relief sprays, creams, roll-ons, etc., with the dosage and instructions. This list and all items listed must be turned over to the administrator upon arrival at the S.H.I.P. program in their original containers. There are no exceptions. Failure to comply will result in immediate expulsion from the program.
