Instructions: Clinician rates each item based on the patient’s condition over the past week. Use a scale of 0 to 10 for each item.
1. Apparent Sadness
0 = No sadness
2 = Looks sad but no verbalization
4 = Talks about sadness occasionally
6 = Persistent sadness evident
10 = Continuous sadness with no variation
2. Reported Sadness
0 = Absent
2 = Occasional sadness reported
4 = Frequent sadness reported
6 = Persistent sadness with despair
10 = Expresses unbearable sadness
3. Inner Tension
0 = No tension
2 = Mild unease
4 = Frequent unease or tension
6 = Persistent inner tension
10 = Extreme tension interfering with function
4. Reduced Sleep
0 = No issues with sleep
2 = Mild difficulty falling or staying asleep
4 = Frequent interruptions or reduced sleep
6 = Persistent lack of restorative sleep
10 = Insomnia severely impacting daily function
5. Concentration Difficulties
0 = No issues
2 = Mild difficulty focusing
4 = Frequent lack of focus
6 = Persistent lack of concentration
10 = Total inability to concentrate
Total MADRS Score Interpretation:
0-6: Normal
7-19: Mild Depression
20-34: Moderate Depression
≥35: Severe Depression
