New Homeowners Information Form

Individuals registered on the property title can provide the following information to set up their account and begin the process of obtaining their membership cards.

Once this form is submitted with the new homeowners information, the homeowner must provide either the Certificate of Title, or the completed Purchase Agreement, to the office via email to office@mlra.ca . Once this documentation is recieved, the homeowners must pay a $21.00 one-time new member registration fee for the household, before membership cards are issued.

Adult children, boarders, and caregivers 21 years of age and older living in the member's residence may be issued cards once Government issued PHOTO ID (drivers license or Alberta Identification card are accepted) reflecting the McKenzie Lake Address is provided.

For children who are between 12-15 years of age, a Children's Waiver form must be submitted by the parent/guardian for a membership card to be issued. Children under the age of 12 years old are not issued membership cards.
