This form is to be completed by the registered homeowner(s) of the McKenzie Lake property who wish to transfer their membership privileges to their current tenants.
Requirements

In order for tenants to receive membership cards for lake access, this form must be completed by the homeowner. The tenant OR the homeowner is required to provide the office with a copy of the lease/tenancy agreement via email to office@mlra.ca OR through a file uploaded along with this form . The tenants must provide their drivers license with the McKenzie Lake address on it before membership cards are issued.

I I acknowledge that by submitting this form, my lake access privileges will be transferred to the tenants listed above, and my lake access will be suspended for the duration of their lease.I understand that lake access will only be granted if MLRA annual HOA fees are up to date and in good standing.I acknowledge that as the homeowner, it is my responsibility to provide written notice to MLRA for any updates regarding the tenancy of my property.I understand that by submitting this form, any tenants not listed will be removed from the property. Tenants listed on this form will replace previous tenants as it is assumed they are no longer residing at the property.I acknowledge that as the registered owner, it is my responsibility to provide MLRA with written notice of an alternative mailing address if required.I acknowledge that either I, as the homeowner, or the tenants will provide MLRA with a copy of the signed lease agreement.I have informed the tenants listed above of the rules and required documentation needed for them to receive their MLRA membership card.
