San Antonio Animal Care Services, Protect Your Pet, Spay Neuter Network, 24Pet, and Petco Love will have San Antonio's FIRST MEGA free pet vaccination event on Sunday, March 30th, 2025 at the Wheatley Heights Sports Complex. (details will be sent by email once pre-registered and appointment confirmed)

REGISTRATION OF ALL PETS IS REQUIRED.

4 pets MAX per vehicle/ per registration.

All cats MUST be in a carrier.

This form will not save if not completed. Please have prepared the name, gender, breed, age, color, and picture of your pet(s) before continuing.

The following information is required in order to have your pet(s) vaccinated and microchipped. This information will not be distributed to solicitors. 

Please review and sign consenting to the information below so we can set up your appointment:

• I am the owner of the animal(s) listed in this form, responsible for them, and have the authority to execute this consent. I authorize the veterinarians and/or their assistants to vaccinate my animal(s).
• My animal(s) is/are not pregnant and do not have a chronic medical condition. 
• My animal(s) is/are currently healthy and have not shown signs of illness (coughing, sneezing, vomiting, diarrhea, lethargy, loss of appetite, etc.) within the last 14 days. 
• My animal(s) has/have not bitten a person within the last 10 days. 
• I understand that vaccinations are generally safe and can protect my animal(s) from several diseases, but vaccine reactions can occur in some animals at no fault of the veterinarian. Vaccine reactions may include mild fever, local swelling/soreness, facial swelling, hives, or, in severe cases, anaphylactic shock. I am encouraged to discuss any concerns I have about vaccinations and the associated risks with the attending veterinarian and/or assistant prior to vaccination. Should my animal(s) become ill due to vaccines, I agree to treat any medical concerns/conditions or vaccine reactions at a full-service veterinary clinic or emergency clinic, and this will be my sole financial responsibility. 
• My animal(s) does/do not have a history of vaccine reactions. 
• I understand that my animal(s) is/are not receiving a full and complete exam today and that a comprehensive exam should be performed at least yearly at a full-service veterinary clinic. 
• I understand that if my pet(s) receive(s) vaccination services and do(es) not have a microchip, they will be microchipped and registered with the information provided in this form.
• I agree to indemnify and hold harmless the City of San Antonio Animal Care Services, its employees, and affiliates, including the attending veterinarians, from any and all liability arising out of the performance of all procedures referred to above. 
• I acknowledge that there is a risk of scratches, bites, and/or animals escaping in a mobile vaccination setting and will not hold the City of San Antonio Animal Care Services responsible for this or any illness, injuries, and/or loss to myself, my animal(s) before, during, or after the vaccine clinic.
• I authorize the City of San Antonio Animal Care Services to use media taken for any legal use. I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
