I, {parentlegalGuardian}, the parent or legal guardian of {childsName}, give my permission for my child to volunteer at the York County All On Board Talent Show on March 13th at Elevation Riverwalk. In an emergency situation, I give permission for my student to receive medical care and understand efforts will be made to contact above guardian.
1- I understand and consent that photos and videos of my child participating in this event may be used for present and future promotions of All On Board.
2- I have read this “release and assumption of risks agreement” with respect to the Talent Show and understand and accept both the terms of the risk I am assuming. I forever RELEASE, WAIVE, and DISCHARGE, and COVENANT NOT TO SUE York County All On Board, it’s board members, executive director, officials, employees, representatives from, against or on account of any and all demands, claims, actions, suits, damages (property and personal), losses, liabilities, injuries (including serious bodily injury and death), sickness, illness, costs and expenses, (Collectively “claims”) arising directly or indirectly out of or related to the event or experience. I voluntarily sign this agreement to evidence by agreement to its terms and conditions.
