I HEREBY ASSUME ALL RISKS for myself and my dog(s) related to participation in this activity or event. I acknowledge that this Accident Waiver and Release of Liability will be used by the event holders, sponsors, and event location, and that it will govern my actions and responsibilities during this activity or event.
The powder used in this photo session is made with food-grade and cosmetic-grade ingredients. The powder consists of cornstarch and natural dyes and has been certified in the United States and tested/certified as FDA-approved and non-dust explosive by OSHA. Although the colored powder should not be consumed, it is also not intended to be inhaled in large quantities. During the session, dogs are always directed to run away from the powder, and owners are instructed to call their dogs out of the dust cloud.
While no dog has ever experienced a skin reaction to the powder, it is recommended that owners use a blow dryer to remove all loose powder immediately after the photo session. Some colors may stain light-colored dogs; if staining occurs, it should wash out after a few washes.
While it is understood that accidents may happen, any damage to equipment, including cameras, caused directly by the owner’s dog(s) shall be the sole responsibility of the owner.
I ACKNOWLEDGE the risks associated with the powder used during this photo session and voluntarily agree to assume all such risks. I HEREBY INDEMNIFY, HOLD HARMLESS, AND AGREE NOT TO SUE the entities or persons mentioned in this waiver from any and all claims or liabilities arising out of participation in this activity or event, whether caused by negligence or otherwise.
I acknowledge that this activity may involve physical activity for both myself and my dog(s) and may carry a risk of injury. I CERTIFY THAT I HAVE READ THIS DOCUMENT, FULLY UNDERSTAND ITS CONTENTS, AND AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A LEGALLY BINDING CONTRACT. I SIGN THIS WAIVER OF MY OWN FREE WILL.
