    1. If this is an emergency: 

    • Dial 000 then Call your Coordinator (if no answer, call 0438 935 818) then complete the Incident Report below
    • Follow directions from Emergency Services 

    2. If this is not an emergency: 

    • Follow your Chain of Command listed on SupportAbility 
    • Complete the Incident Report below, submit it
      • If immediate response is required, HR will call you. 
    • Otherwise, your report will be sent to the Coordinators and they will contact you
    • Journal in your activity notes that you have completed an Incident Report
