Leave Notification 缺席通知

Student member who wishes to take leave from Gifted Education Fund: Off-School Advanced Learning Programmes (OSALPs) class or early leave should notify the corresponding the HKAGE by submitting this form. Please note the following remarks for the leave application:
學員如欲申請缺席或早退「資優教育基金：校外進階學習課程」的課堂，須填妥此表格及將證明文件交予學苑。學員請注意以下事項：
- Except sick leave or emergency circumstances, student members should submit this Leave Notification Form with supporting document before the lesson. The HKAGE and the Programme Provider may exercise exemption if students are absent for being the school representative in international competitions. Such exemption may be granted only if the students can provide sufficient documentary proof. The HKAGE and the Programme Provider will make final decision on granting the exemption.
除了病假及緊急情況外，學員應於課堂前填妥並提交此「缺席通知」連同證明文件（如有需要）。如學員因代表學校出席國際比賽而缺席上課，可能可獲香港資優教育學苑及課程提供機構豁免紀錄，惟須提供充分文件證明。香港資優教育學苑及課程提供機構對是否豁免學員有最終的決定權。
- Late attendance or early leave for any session will be recorded and student members will be marked absence “Absent” if it is over half of the session.
上課遲到及早退將被紀錄，遲到或早退超過課堂一半時間將被視為缺席。
- For the programme delivered by an external Programme Provider which does not take place in the HKAGE, students should notify his/her leave to the HKAGE as well as the Programme Provider. Please refer to the instruction for the particular programme.
若課程由學苑以外的課程提供機構協辦及在學苑以外的場地進行，學員除通知學苑外，並須通知有關課程提供機構。請留意個別課程的有關安排。
- Leave notification will be remarked in the attendance record of the programme, student members should note that the record may affect the awarded certificate if the overall attendance requirement is not fulfilled.
缺席通知會記錄在課程的出席紀錄中。學員須留意所屬課程要求的出席率，出席率不足的同學將不會獲頒課程證書。
- The HKAGE can terminate or cancel a session due to inclement weather conditions or other unforeseeable reasons. (Click here for Inclement Weather Arrangement). The session cancelled will be excluded in the calculation of attendance regardless whether any make-up session will be arranged.
學苑可因惡劣天氣或其他意料之外的原因提早下課或取消上課。（按此了解惡劣天氣安排）不論補課與否，已取消的課堂將不會計算在出席率之內。
