    Thank you for partnering with One One Cocoa to support the development of Caribbean student-athletes. Please complete this form to nominate up to five student-athletes from your institution for consideration in the Cocoa Collective Success Initiative (CCSI). This three-month program is designed to empower Caribbean student-athletes to thrive within and beyond their college athletic careers.

    Qualifications: The nominee(s) must 

    • Caribbean Heritage: have origins from a Caribbean country.
    • Recent Enrollment: Be currently enrolled as a student-athlete or have graduated within the past semester.

    NOTE: Each nomination form will allow for a maximum of 5 student-athletes to be nominated.
