2. Liability Waiver and Assumption of Risk

I, the undersigned, acknowledge that participation in physical activity may involve risks, including the risk of injury. This includes participation in live and pre-recorded instructional videos, as well as any in-person fitness activities. I voluntarily assume all risks and liabilities associated with my child's participation in these fitness activities, whether delivered in person, via live video, or through pre-recorded content.

By signing this waiver, I hereby release and hold harmless Bionic Fitness, its employees, agents, and instructors from any and all liability, claims, demands, or causes of action arising out of or in connection with any injury or accident that may occur during the fitness classes, including those conducted through live or pre-recorded videos.

3. Medical Consent / Health Information Form
