We greatly appreciate your assistance and commitment to serve our community. Our insurance policy requires that we have an accurate record of all Participants. This is an annual form where you agree to release La Cosecha, Inc. of all liability while working in their installations. This form is in effect for one year from the signing date.

This Release and Waiver of Liability (the "Release") executed on:

(the "Participant") in favor of La Cosecha, Inc. a non-profit organization, and their directors, officers, employees, and agents.

The Participant hereby freely, voluntarily, and without duress executes this Release under the following terms:

Release and Waiver: Participant does hereby release and forever discharge and hold harmless La Cosecha, Inc. and it successors and assigns from all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Participant's activities during event.

Participant understands that this Release discharges La Cosecha Inc. from any liability or claim that the Participant may have against La Cosecha Inc. with respect to any bodily injury, personal injury, illness, death, or property damage that may result from the Participant's activities associated with or at La Cosecha Inc, whether caused by the negligence of La Cosecha, Inc. or its officers, directors, employees, or agents or otherwise. Participant also understands that La Cosecha, Inc. does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness.

Medical Treatment: Participant does hereby release and forever discharge La Cosecha, Inc. from any claim whatsoever which arises or may hereafter arise because of any first aid, treatment, or service rendered in connection with the Participant's activities at La Cosecha Inc.

Assumption of Risk: The Participant understands that the activities include work that may be hazardous to the Participant, including, but not limited to, loading and unloading boxes of food, lifting heavy boxes, etc.

Participant hereby expressly and specifically assumes the risk of injury or harm in the activities and releases La Cosecha, Inc from all liability for injury, illness, death, or property damage resulting from the activities.

Insurance: The Participant understands that, except as otherwise agreed to by La Cosecha, Inc. La Cosecha, Inc. do not carry or maintain health, medical, or disability insurance for any Participant. Photographic Release: Participant does hereby grant and convey unto La Cosecha, Inc. all rights, inall photographic images, video and/or audio recordings made at La Cosecha, Inc. during the Participant's activities for promoting our resources to the community.

Other: Participant expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Oklahoma and that this Release shall be governed by and interpreted in accordance with the laws of the State of Oklahoma. Participant agrees that if any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

IN WITNESS WHEREOF, Participant has executed this Release as of the day and year first above written.

If the Participant is under the age of 18, a parent or legal guardian must sign.
