  • Meals on Wheels - NAPIS Form

    National Aging Program Information System (NAPIS) data is required by the federal government and is used by the South Dakota Adult Nutrition Program to track how many S0uth Dakotans receive meals. The data collected helps the Administration of Community Living determine how to allocate funding based on the aging population through the country. All collected information is kept confidential.
  • If you are under the age of 60, you must contact Dakota at Home for eligibility. You can either call them at 1-833-663-9673 or visit their online referral page to complete the process: Dakota at Home Referral Form.
