The undersigned pet owner ("Owner") hereby grants K. Lamar Goods and Services, doing business as Kailey's Dog House ("Service Provider"), permission to care for their pet(s) during the requested boarding dates. The Owner acknowledges and agrees that Kailey's Dog House is authorized to provide the necessary care, supervision, and services for their pet(s) as specified in the contract.

1. I confirm that I own the pet or I was given authority by the owner for taking ownership of the pet.
   
   
2. I assure Kailey's Dog House (K.Lamar Goods & Services LLC) that this pet is currently in good health condition.
   
   
3. I confirm that my pet has a complete and updated vaccinations.
   
   
4. I acknowledge that the pet must have a safety collar or harness.
   
   
5. I confirm that my pet doesn't have any flea or any communicable disease.
   
   
6. I release Kailey's Dog House (K.Lamar Goods & Services LLC), its employees, and owners from any liabilities but not limited to injury, sickness, damage, accident, or death while in the vicinity.
   
   
7. If ever I am unable to pick up my pet on time, Kailey's Dog House (K.Lamar Goods & Services LLC) will make multiple attempts to contact the owner and get authorization for the pet to stay with Kailey's Dog House (K.Lamar Goods & Services LLC). I understand that an additional late pick-up fee may be accrued.
   
   
8. If my pet needs medical attention, I authorize Kailey's Dog House (K.Lamar Goods & Services) to have my pet seen by the nearest and first available veterinarian. I understand that I am liable for any and all veterenarian costs, including for any preexisting or dormant conditions my pet had prior to being in the care of Kailey's Dog House (K.Lamar Goods & Services LLC).
   
   
9. I confirmed that all information I entered in this form is true and accurate
