Daily FUEL Pack for Your AthleteThis purchase includes (4) Daily Fuel Packs. Each pack includes (1) healthy sports drink, (1) pre-packaged snack, and (1) whole foods snack PLUS (1) little "goodie/prize" served daily to your athlete during free time. Our food and drinks are carefully selected to fuel little athletes and do NOT include food dyes or added sugars.

Concession "Fuel" CreditWe will not be handling cash this summer - all concession credit will be held on your child's account. We pride ourselves in providing FUEL for our athletes! Healthy snack and drink options are carefully selected and do NOT contain food dye or added sugars. $6/day recommended for concessions. Remaining balance at the end of the week will be donated to Empowered Athletes Sports Camp.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Empowered Athletes - All Girls Sports Camp (Our Beautiful Ventures LLC) during the selected camp. In exchange for the acceptance of said child’s candidacy by Empowered Athletes - All Girls Sports Camp (Our Beautiful Ventures LLC), I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Empowered Athletes - All Girls Sports Camp (Our Beautiful Ventures LLC) and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Empowered Athletes - All Girls Sports Camp (Our Beautiful Ventures LLC) including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities offered at camp. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

I hereby grant the Empowered Athletes - All Girls Sports Camp permission to use my child's likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications and social media, without payment or other consideration.*
