Does the artist have any allergies, chronic illness, skin conditions like eczema medical conditions? (We will be face painting) If yes, please describe. (Especially Food allergies, food dyes) Friday is a special day each camp and food will be provided. Are there any special accommodations needed?*

Does the artist have any history with seizures? We have flashing lights and will use black lights on certain days when we paint glow in the dark projects. Please be detailed in explanation. We will not use these lights if it will cause medical issues.*

🎨Limited seats available (only 10-14 students will be permitted), these will be held in my private studio. Due to the popularity of art camps, be sure to REGISTER early to not miss out, as spaces fill up fast! Fun, Materials and instructions will be provided!!

🎨WHAT: We can do canvas painting, splatter painting, wood painting, multi-media art, ceramics, drawing, and watercolor! FUN arts and crafts!

🎨WHAT TO WEAR: Have your child(ren) wear play/paint clothes. Most of our projects include paint and it will stain. Summer clothing for play outside as well.

🎨SNACK AND LUNCH: Bring a water bottle. We will have outside play time. Pack a snack and lunch for Monday thru Thursday. Friday a snack only, we will have Pizza or Chicken Nuggets with sides!

🎨WHEN: Art Camps are Monday thru Friday, 1 pm - 6 pm each week!

🎨AGE POLICY: Art Camp is open to ages 5 to 12 years of age.

🚶‍♂️WALK- INS: We do not take Walk-ins. We prepare for each child well in advance. Paid Invoice and completed form reserves your child's spot!

🚗BEFORE and AFTER CARE: Before care has to be arranged with Tammy ahead of time and will be offered very limited. Drop off is between 12:45 PM and 1:00 pm. The pick up is at 6:00 pm.

🎨BEHAVIORAL POLICY - It is very important to let us know if there are any medical or behavioral issues with your child that will affect his/her participation and how we interact with him/her. We want to give every child the best experience possible in a group setting!

If we've taken corrective action, and the behavior does not improve, we will contact parents/guardian in attempts to correct the behavior. If it is still not corrected, parent/guardian will be called and asked to pick up their child. No exceptions. PLEASE NOTE: Elimination from the camp for any amount of duration (half day, full day, week, etc.). There will be no refunds in this instance.

🎨CANCELATION/REFUND POLICY - Cancelation with full refund is given only up to the 30 days before Camp Session. Cancelations within the 30 day window before day one of Art Camp are non-refundable, however we will provide you with DIY art kits for the projects to be picked up on Friday!

Informed Consent and AcknowledgementI hereby give my approval for my child’s participation in any and all activities prepared by Livelovepaint Studio/Tammy Amsler during the selected camp. In exchange for the acceptance of said child’s candidacy by Livelovepaint Studio/Tammy Amsler, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Livelovepaint Studio/Tammy Amsler and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.In case of injury to said child, I hereby waive all claims against Livelovepaint Studio/Tammy Amsler including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all art or playground/sports activities, including basketball. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.Permission is also granted to the or Livelovepaint Studio/Tammy Amsler and its affiliates including Directors, Teachers, Principals or Coaches to provide the needed emergency treatment prior to the child’s admission to the medical facility.Release authorized on the dates and/or duration of the registered season.This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.Confirmation: BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
