- Is released by me from any liabilities resulting from the donated goods.
- Is held harmless from any claims or obligations regarding the donated goods.
- Offers no express warranties in relation to the gift of goods.
I certify that I am a member of the household listed above, and, on its behalf, I have applied for assistance through ICNA Relief’s food distribution program. I verify that all information provided regarding my household is accurate to the best of my knowledge. I also designate the signee as an authorized representative of my household, certifying that their information is accurate to the best of my knowledge. The authorized representative is permitted to pick up goods on behalf of the client until re-certification is required.
