I am aware that I am potentially choosing to receive a chemical service. I understand that this can cause an allergic or otherwise negative reaction. I understand these risks and if I have any concerns I will seek medical advice prior to service. Further, I grant my chosen stylist permission to provide services to me. I do not hold her responsible for any or all adverse health or other negative reactions that may occur.

I understand that the process of a chemical service has the potential to damage my clothing or other personal property. I will not hold my stylist liable for any of these damages.

It is common that the stylist will take pictures of her work to post on social media pages, for use of advertising. Do you give permission to use photos of you/your hair for this purpose?

I understand and agree that payment is due at the time of service. I understand that there will be no refunds given for services I receive.

I will not hold the stylist liable for any injuries, adverse reactions or damage to personal property that may occur on or around their premises.

There is a cancellation fee of 35% of your scheduled service if you are to cancel within 48 hours of your appointment, or if you fail to show up to the appointment with no communication. The fee will be charged to the card on file.
