    Final Report Fiscal Year 2025

    The purpose of the final report is to provide the York County Arts Commission (YCAC) with a written record of your grant-related activities, accomplishments, and challenges that arose during the grant term. This report will be used for grant management, assessment, and evaluation of your project’s impact on the arts in York County. It is important you adhere to the instructions outlined in this document and provide us with the requested information including copies of all receipts, programs, and photographs.

  • List materials and supplies purchased with grant funds. THE TOTAL OF RECEIPTS CANNOT BE LESS THAN THE GRANT AMOUNT RECEIVED
  • Attach a copy of project programs, photographs, or similar materials from the projects supported by the YCAC. These items must be attached or included with your final report form submission.
