Volunteer Background Check/Child Protection Training Request Form

For volunteers assisting with financial duties (offering, etc.) or children's ministry.
Offering Team

Staff and volunteers age 18 or older who have completed a background check are eligible to assist with offering (with approval of the lead pastor, completion of appropriate offering procedure training, and following all Equipt cash handling policies and procedures.)

Children’s Ministry

Staff and volunteers age 18 or older who have completed both a background check and the online child protection training are eligible to help with children’s ministry (with approval of the children’s ministry director, completion of appropriate children’s ministry training, and following all Equipt child protection policies and procedures.)

Youth volunteers do not participate in background checks or the online child protection training. They are approved to serve in children's ministry at the discretion of the children's ministry director, following the guidelines outlined in the Equipt child protection policies and procedures.

- Volunteers will be emailed a background check consent form from message@mobilizemyministry.com.
- Volunteers who will serve in children's ministry will also be sent an online child protection/abuse awareness training from info@equiptteam.org to complete.
- You will be notified when the volunteer is cleared to serve.
- This form allows for up to four volunteers to be submitted. You only need to fill in the boxes for the number of volunteers you are submitting. All others can be left blank.
