In consideration for being accepted by LIFE CHURCH to participate in the event(s) during the years 2025–2026, I, the undersigned, do hereby release, forever discharge, and agree to hold harmless LIFE CHURCH, its directors, employees, and agents, from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses of any nature whatsoever, which may be incurred by the undersigned or the participant while participating in the above-described event(s).
I acknowledge and accept the following:
Assumption of Risk:
I, on behalf of myself and my child (if under the age of 18), assume all risks of personal injury, sickness, death, property damage, or expenses as a result of participation in recreation, work activities, or any other activities related to the event(s).
Medical Authorization:
As the parent(s) or legal guardian(s) of the participant (if under the age of 18), I grant permission for the participant to receive medical treatment, including being taken to a doctor or hospital.
I assume full responsibility for all medical expenses incurred during or as a result of participation.
Emergency Return:
Should the leadership of the event(s) determine it necessary for the participant to return home due to medical reasons, disciplinary actions, or otherwise, I agree to assume full responsibility for transportation arrangements and all associated costs.
Indemnification:
I agree to hold harmless and indemnify LIFE CHURCH, its directors, employees, and agents, against any liability sustained as a result of negligent, willful, or intentional acts of the participant, including related expenses.
Acknowledgment of Age (for participants 18 and older):
I, being eighteen years of age or older, accept the terms of this release for myself.
