2025 FBS Golf Tournament Registration

Florida Biomedical Society's 2025 Annual Golf Tournament, Disney’s Palm Golf Course Date: Friday, December 5th, 2025 Time: 8:00 am Shotgun Start(Shuttle Transportation leaves Disney’s Beach Club Resort at starting at 6:30am. Shuttle pick up at front of Beach Club main entrance. Must state “Needs Transportation” when registering) Rate per Player: $225.00 Includes: Includes: Private shuttle transportation, green fees, use of electric golf cart w/ GPS, pre-round practice balls, lite breakfast, box lunch and prizes. ***This event is for paid, registered golfers only. No other guests will be allowed on the course or on the shuttles***

Instructions: You may register as a single player or up to four persons which will complete a team. Note: Individuals will be placed in a foursome by tournament organizers. Please enter the name of each player below, a minimum of one player is required to complete form. Multiply the number of player(s) by $225.00 each and enter the total amount in the box labeled "total Amount due" and submit. You will be than sent to our PayPal site for payment.
