JLC Training Guidelines For Clients
  • Small Group Training Client Agreement

  • An initial consultation will determine your fitness requirements and help you to set appropriate goals. Your personal aims and lifestyle information will then be used to design an appropriate training program.

    The following when signed will constitute the full agreement between Joanna Watkinson (herein referred to as "trainer') and client

    The trainer will act as a Personal Trainer in the development of an exercise programme during the agreed sessions.

    In the consideration of the service, the client shall pay in advance according to the session rates charged by the trainer. Refunds are at the trainer's discretion.

    You will be required to have an initial assessment with your Trainer, unless an assessment is deemed unnecessary by the Trainer.

    All appointments must be pre determined with your Trainer 

    Suitable workout clothing and footwear must be worn.

    Please inform your Trainer of any medical conditions you have, or any medication you may be taking. If unsure, please ask.

    The client agrees, if for any reason they are unable to attend a session, it will continue as planned for the rest of the group members. No session will be owed to them. This includes holidays, illness, other commitments etc.  IF there is availability in an alternative session/group that week the client will be offered this option.


    The client agrees payment for services will be collected by monthly direct debit wherever possible.

     

    The client shall pay in advance  according to the  agreed session rates charged by Joanna Lynne Coaching. This payment covers all sessions for the month. 


    The client acknowledges the coach will provide them with a small group coaching session at the agreed frequency, at the agreed time/s on the agreed day/s for a minimum of 46 weeks per calendar year (1st Jan - 31st December). 

    This entitles the coach to 6 weeks per calendar year when sessions are not provided - this has been reflected within the monthly membership prices.


    When the trainer goes on holiday, there will be no in person coaching provided. But you will be provided with a clear plan of action to maintain the momentum of your health and fitness journey. This will be arranged at least one month in advance before I take leave.

     

    Cancelation agreement between trainer and client

     

    1. The client agrees to an initial 3 month minimum contracted period after which the client is required to give one full calendar month's notice of any cancellation or amendments to the payment plan(s) in relation to Joanna Lynne Coaching. Any payment due within this period remains payable.

     

    2. The trainer shall provide the client with no less than one month's notice where possible  of any pre-planned absence affecting sessions.


    3. The trainer shall provide no less than one calendar month's notice where possible of any termination or adjustment to the clients agreed training session time or structure

  • Personal Injury Waiver and Photography/Media Consent


    I understand that my participation in exercise programs, classes or events and any related activity conducted by Joanna at Joanna Lynne Coaching is voluntary and at my own risk.


    I am aware that exercise can be physically stressful and, in certain instances, even result in injury or cause of death. The levels of exercise that I will perform will be at my own pace, based upon my cardiorespiratory fitness, muscular strength, movement capabilities and endurance levels.


    I hearby state that I will inform the trainer of any symptoms (E.G. fatigue, shortness of breath, chest discomfort, any pain/discomfort/concern for my safety) during my participation in exercise. If I have high blood pressure, diabetes, a heart condition, any existing injury, recent surgery or if I am taking any prescribed medications that could affect my performance, I will inform the trainer prior to participating in any exercise.

  • Joanna/Joanna Lyne Coaching will not be liable for any injuries or damage arising from participation in training to the undersigned. It is agreed that the trainer/Joanna Lynne Coaching shall not be responsible or liable to the undersigned for articles lost or stolen whilst training with the trainer/Joanna Lynne Coaching.


    By taking part in training with Joanna Lynne Coaching (including but not limited to personal training, group training or training events) I agree that full rights to the use of images resulting from photography/video filming during my training, and any reproductions or adaptations of the images can be used for fundraising, publicity or other purposes to help achieve the aims and objectives of Joanna Lynne Coaching. 


    This might include (but is not limited to), the right to use them in printed and online publicity, social media, press releases and funding applications. 

  • I do/do not consent to Joanna Lynne Coaching photographing and/or filming me and for these images or videos to be used for marketing and other promotional uses, including (but not limited to) it’s use on JoannaLynneCoaching.com and its associated social media sites.
  • Declaration of understanding

  • I, the client, agree that I fully understand and agree to abide by the above rules and regulations. I understand that I should inform the trainer of any medical condition and that I may be advised to visit my doctor prior to commencing the sessions.

    I acknowledge that I have either had a physical examination and have been given my doctors permission to participate, or that I have decided to participate in the activity and/or use the equipment or machinery without the approval of my doctor and do hereby assume all responsibility for my participation and activities.

    I release my trainer (Joanna Watkinson) from any liability for personal injury or other damage I may sustain whilst engaging in any exercise programme suggested by my trainer.

    Thank you for your cooperation, I hope you enjoy your Personal Training sessions.

    I acknowledge receipt of this agreement and confirm acceptance of the terms and conditions contained therein.
