Liability Waiver
1. ACKNOWLEDGMENT OF RISKS I understand that participation in football involves inherent risks, including but not limited to:

Physical contact and collision
Sprains, strains, fractures, concussions, or other injuries
Weather-related injuries
Accidents during practice, games, training, or travel
I acknowledge that these risks can result in serious injury, disability, or even death.

2. ASSUMPTION OF RISK I voluntarily choose to participate (or for my child to participate) in football activities and assume all risks, whether known or unknown, associated with participation.

3. RELEASE OF LIABILITY In consideration for being allowed to participate in football activities, I hereby release, discharge, and hold harmless:

The coaching staff
Team volunteers and organizers
The league, school, facility, or organization
All affiliated staff, members, officers, and agents
from any and all liability, claims, demands, or actions arising out of or related to any injury, loss, or damage that may occur during participation.

This release applies to injuries caused by ordinary negligence.
(Note: Many states do NOT allow release of liability for gross negligence or willful misconduct.)

4. MEDICAL CONSENT I authorize the staff, coaches, or volunteers to obtain emergency medical care for me/my child if deemed necessary. I agree to be financially responsible for any related medical expenses.

5. INSURANCE ACKNOWLEDGMENT I acknowledge that it is my responsibility to maintain adequate medical insurance for myself/my child. I understand that the organization may not provide such insurance.

6. COVID-19 / COMMUNICABLE DISEASE WAIVER (Optional)I acknowledge the risk of exposure to communicable diseases and agree that the organization and staff are not liable for any illness contracted during participation

GB Elite Flag Football & Boundless Media Media Release & Waiver

I, the parent or legal guardian of the minor named below, hereby grant GB Elite Flag Football and Boundless Media, including their representatives, permission to photograph, record, and/or video my child during team practices, games, tournaments, and related activities.
I understand that these images or recordings may be used for lawful purposes including, but not limited to: team and league promotion, social media posts, website content, marketing materials, and highlight videos.
I acknowledge that these images and recordings may be used without compensation, and I waive any right to inspect or approve the finished product or the manner in which it is used.
I release and hold harmless GB Elite Flag Football, Boundless Media, and their respective coaches, staff, volunteers, contractors, and affiliates from any claims, demands, or liability related to the use of my child’s likeness, name, image, or voice.
This authorization remains in effect for the duration of my child’s participation with GB Elite Flag Football unless revoked in writing.
