PLEASE NOTE USE OF OIL BASED PRODUCTS AROUND EYES SHOULD BE DISCONTINUED FOR 48 HOURS AFTER YOUR EYELASH EXTENSION SERVICE. It is also recommended to avoid all oil based products around your eyes for as long as you wear your lashes. Oil based products, waterproof mascaras and liners will loosen the adhesive and your lashes will not last long.

How do you usually sleep? Please note you will lose more eyelash extensions on the side on which you sleep. Sleeping on your stomach will affect them the most and it is important to refrain from sleeping on your stomach for the first 2 days after your service to allow the adhesive to set properly.

Are you allergic to adhesives ( glues, tapes, band aids, etc)? This service may use adhesives tapes, glues and gel pads that may cause an allergic reaction.

Have you had Lasik Surgery in the past 4 months? Eyes may have sensitivity to eyelash extensions and products used for prepping the eye area (gel pads, glues, etc.)

Booking Deposit - Required for new appointments. Deposit is half of a full set eyelash extensions, $20 for lash lift or any brow service. DEPOSITS ARE NON REFUNDABLE. If appointment is rescheduled 48hrs in advance deposit may be transferable to new booking date. Deposit may be sent through cashapp or Venmo.

Late policy - Please arrive on time with no extra guest. There is a 10 minute grace period. If you arrive more than 10 minutes late to the scheduled appointment, client will need to reschedule with added late fee* to next appointment.
(*$10 late fee)

Appointment Confirmation - please confirm appointment through booking link. If Any changes need to be made to your appointment, please notify me at least 24 hours in advance in order to avoid a cancellation fee.

Reoccurring clients who cancel, show up late, or do not properly take care of lashes may no longer be accepted. No call/no shows will be charged a cancellation fee and will need to prepay in full for next appointment if wanting to book again (*cancellation fee is half the price of service).

Appointments: please arrive on time and message me upon arrival before coming to the door. There is no waiting area and I may need time in between clients to clean/ set up for next appointment.

No public restrooms (may use if needed) please use restroom before coming to appointment. Please come to appointment with no makeup around eye area, do not drink caffeine prior to appointment, and prepare to lay down for 2+ hours. Silence cellphones, no chewing gum and keep moving to a minimal.

Payments - all card payments will be charged a 2.8% fee

I understand that eyelash extensions require ongoing maintenance. Refills are recommended approx. every 2-3 weeks. I understand if I go beyond this recommended time and have less than 40% lashes remaining I may need a full set and will be charged a full set price.

All clients must be the age 18 or over to receive services. Clients under the age of 18 must have a parent present or have parental consent. I understand the risks associated with having artificial lashes applied to and/or removed from my existing natural lashes, and that not withstanding the aftercare following the application or removal of these products, there is still risks associated with the procedure and products itself, which include without limitation, eye irritation, eye pain, discomfort, sensitivity, and in rare occasion blindness when improperly handled. All of my questions were answered and I understand the procedure and risks, and I agree and understand that the lash tech professional has no way of knowing if the client is allergic to some of the products or materials used in any procedure. I understand that for any given reason a refund will not be issued. I will seek medical care (at my own expense) and contact my technician immediately if any allergic or adverse reaction occurs to have the lash extensions removed. I also understand that there are more then one technique for applying eyelash extensions to my eyelashes and I will not attribute any liability to the Professional from any and all claims, actions, expenses, damages and liabilities, including reasonable attorneys' fees which might be asserted against them as a result of me having this procedure performed, or my purchase of these eyelash extensions or products. As used in this agreement, the terms Professional includes all of their respective officers, directors, agents, employees, successors and assigns..

I grant permission to use my before and after photos for marketing or examples of my technicians work.

I release my certified lash technician and Standout Beauty from any and all liability associated with this procedure. This service will be performed with the utmost attention to safety, sanitation and proper application using tools and products that the technician has been trained and certified to use. This service has many variables due to lifestyle, moisture, weather, extreme tempatures, natural eyelash shedding and other factors. The technician ( along with my consent form and consultation) will decided if I am a good candidate for this serive to the best of their ability.
