PARENT/GUARDIAN RELEASE AND INDEMNIFICATION AGREEMENT
Having read the information concerning the City of McPherson, Kansas ("City") sports program named herein, Iagree to its rules and format and give my child permission to participate in such a program.
Realizing my child is playing for fun, recreation and personal betterment, I hereby, for myself and for my child, our heirs, personal representative and assigns, waive and release any and all claim for injuries or damages, of any kind of nature, ("Claims") which either I or my child may have against the City or any of its officials, employees, agents, or volunteers for such Claims arising from or related to my child's participation in the program including those Claims which may result from the negligence of any of the above-listed individuals. Without limitation, these risks include the transmission of any illness.
I understand that the City assumes no legal or financial responsibility in case of accident or injury, and Iassume full responsibility for my child's medical expenses and without limitation, waive any Claim for my child' medical expenses against the City and each of the persons named herein.
