IMPORTANT: ALL medications should be sent in the original container. If the medication is a prescription, it must be in the original container from the pharmacy with camper's name on it.

Medications of any kind are not allowed in cabins with campers. Our camp staff will collect medications at check-in, and we will distribute the medication as needed throughout the week.

Medical and Indemnity Release 

I am the natural parent, or managing conservator of the above-mentioned student, a minor. I hereby give permission for my student to participate in all activities of CAMP DEAF 2024.

I agree to unconditionally and absolutely RELEASE, WAIVE, HOLD HARMLESS, AND DISCHARGE CAMP DEAF, its officers, employees, agents, representatives, successors, assigns, and volunteers from any and all responsibility owed to the student, the parent, or their legal representatives, heirs and assigns from any and all claims, losses, expenses, injuries, demands, actions, judgments, and/or causes of action, while the student is participating in CAMP DEAF 2024. I am aware of the risk associated by or with participation in this camp. I voluntarily assume full responsibility for any risk of loss, property damage or personal injury, including death that may result from participation from camp activities. Further, should my student need medical treatment while attending the activities of CAMP DEAF, I grant the authority to CAMP DEAF, its officers, employees, agents, representatives, and volunteers to consent to medical treatment in the event I cannot be contacted. This authorization expressly includes the authority to sign releases on my behalf for medical services and facilities. I promise to assume liability for payment of all such medical services and facility fees and to reimburse CAMP DEAF for any medical expenses that may be incurred on behalf of my student.

 

Visual Production Consent and Release

CAMP DEAF occasionally creates publicity and/or public relations advertisements (“Visual Productions”) as a part of the work of CAMP DEAF. While CAMP DEAF is under no legal obligation to obtain a release for your child/children to be part of these Visual Productions (nor is CAMP DEAF assuming any legal obligation arising out of this Consent and Release), CAMP DEAF is attempting to affirmatively record your approval for your child/children’s participation in these Visual Productions. As a condition of your child participating in this activity, we require that you sign and return this form to CAMP DEAF with respect to your child/children. The agreement below allows you to express your approval for your child/children’s name, picture, art, written work, voice, verbal statements and/or portrait (video or still) (collectively, “Image/Statement”) to appear in CAMP DEAF Visual Productions. These Visual Productions may or may not personally identify your child/children and their Image/Statement may be used by CAMP DEAF in Visual Productions in subsequent years.

 

Consent and Release

The undersigned Parent/Guardian releases to CAMP DEAF the Image/Statement of the child/children listed below, and the undersigned consents to CAMP DEAF’s use of the Image/Statement as a part of Visual Productions. The undersigned Parent/Guardian understands and agrees that: No monetary consideration shall be paid by CAMP DEAF for use of the Image/Statement; The undersigned’s consent and release has been given without coercion or duress; The Image/Statement may be used in subsequent years by CAMP DEAF.

This Consent and Release may be rescinded at any time with written notice to Camp Deaf, 6917 Brentwood Stair Rd, Fort Worth, TX 76017.

I acknowledge that I have read and understand this waiver and release and all its terms, and my signature below represents that understanding. I freely relinquish legal rights and I have had the opportunity to obtain any and all counsel if needed. By my signature, I understand and accept this agreement in full. Furthermore, it is understood that a copy of this form is treated as authentic and binding as the original, and my electronic signature of this form is treated as authentic and binding as my physical, original signature. 

Student: If you agree and are willing to comply with all of the expectations of the Code of Conduct at Camp Deaf, please sign at the bottom of the page.

Parent/Guardian: Please sign the bottom of this form to show your intent to support the implementation of this Code of Conduct in regards to your child.

As an student attending Camp Deaf, I agree that:

• I will participate in the camp program, display a positive attitude, and conduct myself appropriately at all times.
• I will respect all campers, advisors, and staff. I will follow instructions from all adults and camp staff.
• I will not use a cell phone during camp programs and activities.
• I will follow the dress code.
• I will not enter a cabin of the opposite sex or be on the wrong side of the camp.
• I will not enter any other cabins or buildings that I am not assigned to.
• I will remain in my assigned cabin after curfew.
• I will not bring highly valuable items to camp.  If I do bring valuables, I accept full responsibility for those items.
• I will not use language or behavior that is obscene, violent, or racially or sexually inappropriate.
• I will not possess or use tobacco products, alcohol and/or drugs.
• I will not possess or use firearms, weapons, pocket knives and/or firecrackers.
• I will respect camp property by keeping the facilities clean, not creating graffiti, and not using items such as water balloons, shaving cream, and toilet paper for destructive purposes. I accept responsibility for damages I cause. 

I understand that failure to meet with these standards may result in disciplinary action, including, but not limited to, sitting out from certain activities, calling parents/guardian, and ultimately arranging for my transportation home from camp (at my parent/guardian's expense).

Student Commitment:

I have read and understand the Code of Conduct above. I agree to abide by it for the safety and enjoyment of myself and of other campers. I understand the consequences of failing to meet these guidelines.
